Celebrating small triumphs

“I’ve never seen her smile so much.” A niece of a case coordination client shared with our staff, after we began to work with her aunt. Her aunt is suffering from cancer and recently had to admit her husband of 64 years to a nursing home. Together they had been a team taking on life and the world. Without him she needed a little extra help to take on life’s challenges. SOC has been providing her the extra hand that she needs.

As a society, we put a great value on doing more things faster. Celebrating the small things in life requires slowing down and paying attention to how another person really feels. Our case coordination department took the time to understand and help 116 seniors that needed extra attention. They invested over 1000 hours getting to know them and helping to empower them.

One hundred and sixteen very frail residents received services from our Case Coordination program. Through this program, comprehensive nutritional, nursing and psycho-social evaluations are provided in the home of elderly residents who have many problems managing their daily activities. Appropriate services are then arranged to help those persons successfully remain in their own homes. Casework services, family consultations and ongoing, personal contact to build relationships are important components of the Case Coordination service. Referrals come most often from family members or friends, followed by local municipalities and police, home health care agencies, doctors and other health providers and self-referrals. and holiday greetings to shut-ins on Thanksgiving, Christmas and Easter.
